11/14/2022 0 Comments Cataplexy narcolepsyThere were significant differences between N and NpC (including mean sleep latency on MSLT), but none between N and N-hd. Data were compared with those of 12 hypocretin-deficient narcoleptics (N-hd). A new narcolepsy score based on five questions was developed. Assessment consisted of questionnaires, polysomnography (all patients), multiple sleep latency test (MSLT) and human leukocyte antigen typing (in most narcoleptics). Based on suggested and published criteria, we differentiated between narcoleptics with definite cataplexy (N) and narcoleptics without definite cataplexy (possible cataplexy, NpC). We prospectively studied 57 narcoleptics with cataplexy, 56 patients with non-narcoleptic hypersomnia (H), and 40 normal controls (No). The aims of this study were (1) to determine frequency and characteristics of sleep-wake symptoms in patients with narcolepsy with cataplexy, (2) to compare clinical characteristics with results of ancillary tests, and (3) to identify factors that discriminate narcolepsy from other conditions with excessive daytime sleepiness (EDS). She was started on Modafinil and venlafaxine. Her MSLT showed severely reduced sleep latency at 2.6 minutes. Her sleep efficiency was 91.7%, REM latency 154 minutes, RDI 2.7/hour and PLMS 0.6/hour. RESULTS: Her neurological examination was unremarkable. She was taking naps during the day and also dreaming during the naps. She did give history suggestive of hypnagogic hallucinations. She had difficulty initiating as well as maintaining sleep. She did complain of excessive daytime sleepiness occurring for the past year or so. She was unable to vocalize but was aware of her surroundings. During sex with her husband, she noticed that her heart would be pounding and suddenly she would be unable to move her whole body. We report a case of orgasmolepsy where it was not only the presenting symptom but also was not accompanied by any daytime cataplectic events.ĭESIGN/METHODS: We report a 27 year old right handed Caucasian lady referred to the neurology clinic for evaluation of episodes of seizures. #CATAPLEXY NARCOLEPSY SERIES#Poryazova et al, in a series of narcoleptic-cataplectic patients, orgasmolepsy was present in 3 out of 75 patients. Orgasmolepsy (cataplexy during orgasm and sexual intercourse) is a rare phenomenon, which has been described in patients with narcolepsy and daytime cataplexy. The usual triggers include laughter, crying and other high emotional state. OBJECTIVE: To report a case of narcolepsy with the presenting symptom of orgasmolepsy.īACKGROUND: Cataplexy, a hallmark symptom of narcolepsy, is typically triggered by strong emotional state.
